Waiver & Participation Agreement
Assumption of Risk
All sports, including soccer, carry inherent risks. I participate in the sport of soccer because it is physically and mentally challenging, and I acknowledge that I am aware of the risks and hazards associated with soccer participation.
These risks and hazards include, but are not limited to, injury resulting from:
- Vigorous physical exertion, running, and strenuous cardiovascular activity, or from stretching and exerting various muscle groups
- Falls due to uneven, wet, or irregular playing surfaces
- Contact with, or being struck by, other participants, balls, walls, equipment, or spectators
- Playing surfaces or shared equipment, including exposure to bacteria, viruses, or skin irritation
- Improper use of equipment, or equipment failure
- Environmental conditions such as extreme heat or cold
- Injury to the spinal cord or other serious, permanent, or life-threatening injury
I further acknowledge that:
- Injuries sustained playing soccer can be severe, and that physical challenge and occasional anxiety are a normal part of this activity
- I may come into close, incidental contact with other participants during play
- My risk of injury is reduced when I follow the rules established for the game and facility
- My risk of injury increases as I become fatigued
Participant Declaration, Release & Indemnity
Prior to entering Joseph J. Piccininni Community Recreation Centre (or any other facility used for Friday Night FC), I confirm that I am fit to participate and am not aware of any medical condition that would make participation unsafe for me.
I am participating voluntarily in Friday Night FC games, events, and related activities. I understand and accept the risks described above, and I agree to be responsible for any injury, loss, or damage I may experience while participating.
Should an injury or loss occur, I agree to release, indemnify, and hold harmless the organizers from any and all claims, demands, actions, and costs — including those arising from the organizers’ own negligence, to the fullest extent permitted by law — that may arise out of my participation, except where caused by the gross negligence or wilful misconduct of the organizers.
For the purposes of this agreement, “organizers” means: Joseph J. Piccininni CRC, the Friday Night FC event organizer and permit holder, its directors, officers, members, employees, volunteers, officials, other participants, agents, sponsors, and the owners/operators of the facility, and their representatives.
If the participant is under 18 years old, this agreement is not effective, and the participant may not take part, until a parent or legal guardian has separately reviewed and consented to these terms on the participant’s behalf, as prompted below.
Medical & Emergency Information
I confirm that the emergency contact information I provide below is accurate, and I authorize the organizers to use it, and to seek emergency medical treatment on my behalf, in the event I am unable to do so myself.
Acknowledgement
I have read and understood the terms of this agreement. By completing the fields below and signing, I confirm that I am voluntarily agreeing to be bound by its terms.
